You are invited to join the Union Hospital/Greater Lynn Senior Services self-management workshop for adults of any age living with persistent health conditions. Caregivers are welcome to accompany their care partners.

My Life, My Health is one of the many names for the Stanford University Chronic Disease Self-Management Program. A chronic health condition is a health condition for which there is no cure.

This self-management six-week, 2 1/2 hours a week workshop has proven to help participants:

• Set and meet individual goals
• Improve communications with doctors, family and friends
• Handle stress and learn to relax
• Increase energy level
• Manage emotional ups and downs
• Have fun!

Each participant receives a copy of the book Living a Healthy Life with Chronic Conditions and a relaxation CD.

Cost: $25.00. Scholarships are available.
